Fill out both the Team Registration Form and the Team Roster Form.

 

Make check or money order payable to “Chi-Town Softball Classic”.

 

Mail forms and payment to the address provided below.

$400 if received by Monday May 31, 2010

$450 if received by Tuesday June 15, 2010
